the Insurance Appeals Process
If a health insurer refuses to pay a claim or ends coverage, the insured person has the right to appeal the decision and have it reviewed by a third party. The insured individual can ask that their insurance company reconsider its decision. Insurers have to tell the insured person why they've denied the claim or ended the coverage.
